You must be an MD or DO practicing or living in Marion County.
The information submitted in this application will be verified. Applicants authorise other organizations holding information relating to the application, including governmental and regulatory entities, to release that information. Any false or misleading statement may be grounds for denial of membership, or for probation, censure, suspension or expulsion by the medical societies.
Members abide by the AMA Principles of Medical Ethics and the bylaws of the Associations.

- Have you ever been convicted of fraud or a felony?
- Question 2
- Has any action, in any jurisdiction, ever been taken regarding your license to practice medicine? This includes revocation, suspension, limitation, probation, or any other imposed sanction or condition.
- Question 3
- Have you ever been the subject of any disciplinary action by any medical society or hospital medical staff?
